Self -Managed Care is an Alberta Health Services funding program for Clients who wish to take direct responsibility for their own care at home, or Clients with a Legal Representative(s) who, on behalf of Client, wish to take direct responsibility for the Client’s care arrangements at home. Clients or their representatives will be required to set up a separate banking account used only for the purpose of receiving funding from AHS and disbursing the funds to pay for client services, according to the needs assessed. To access this service, contact your local Home Care Case Manager for more information, and from there an assessment will be completed to determine your specific needs and eligibility. If unable to find your Local Home Care Office number, please call 1-855-371-4122 (select the option for North Zone when prompted) and they will direct you to your nearest Community Health Provider. You may refer yourself or someone else (with their consent).
This program provides special limited funding that can be accessed by rural palliative clients and families when they require additional support to remain at home at end-of-life. This funding can cover personal care, respite care, or nursing care. The client and family designate someone they trust to be responsible for submitting the caregiver invoices to AHS, receiving payment, and paying the care providers. The care can be provided by formal caregivers such as healthcare aides, licensed practical nurses, or registered nurses who are self-employed or work for an agency. To qualify you must reside in Alberta, have applied for Alberta Healthcare, maximized AHS homecare services, been assessed by a homecare nurse, can be at home safely with the additional direct care
To access this program, contact your home care case manager and/or palliative care team.
